On September 1, medical device company OrganWatch announced that its organ perfusion monitoring system PerfusAI received FDA 510(k) approval, becoming the first AI-powered donor organ real-time monitoring platform to receive FDA certification.

Organ transplant medicine has long faced challenges in donor organ assessment. Traditional static cold storage only preserves organs at low temperatures without providing real-time insight into metabolic status and functional viability. Ex vivo normothermic perfusion technology allows organs to maintain metabolic activity outside the body, but assessing whether a perfused organ is suitable for transplantation still heavily depends on physician experience.

PerfusAI embeds 12 multimodal sensors in the perfusion circuit tubing, collecting perfusate biochemical indicators (lactate, pH, glucose consumption rate), hemodynamic parameters (perfusion resistance, flow waveform), and organ surface optical characteristics (color, microcirculation status). This data feeds into an AI model trained on 100,000 organ perfusion cases, outputting an organ health score (0-100) and transplant success probability prediction every 5 minutes.

OrganWatch's clinical trial data shows that with PerfusAI assistance, organ discard rates dropped from the traditional 18% to 5%, and one-year post-transplant survival rates improved from 82% to 91%. "A significant number of donor organs were historically discarded due to assessment uncertainty," said OrganWatch's Chief Medical Officer. "PerfusAI helps transplant teams make more accurate judgments, allowing more organs to be utilized."

PerfusAI is priced at approximately $350,000 per device with disposable sensor kits at about $800 each. OrganWatch has partnered with the United Network for Organ Sharing (UNOS) and plans to cover 80% of US organ transplant centers by end of 2029.
