NanoSentry Multi-Cancer Early Screening via Circulating Tumor DNA: One Blood Draw Flags Signals from 14 Early Cancers

GRAIL's second-generation early screening product NanoSentry has completed evaluation in a large-scale NHS pilot in the United Kingdom. Based on circulating tumor DNA methylation patterns, a single blood draw screens for 14 early-stage cancers at 78 percent sensitivity and 99.5 percent specificity.

US early-cancer screening company GRAIL has presented data from its large-scale UK NHS pilot for the second-generation early screening product NanoSentry at the 2026 European Society for Medical Oncology (ESMO) congress. NanoSentry is a multi-cancer early screening test based on circulating tumor DNA methylation patterns; a single blood draw screens for 14 early-stage cancers.

The pilot was conducted jointly by the UK National Health Service and GRAIL and covered 140,000 British citizens aged 50 to 77. Participants were randomized into two groups: one received NanoSentry screening, the other received standard NHS health checks as control. After 12 months of follow-up, the screening group had an early-cancer detection rate 4.2 times that of the control group, and 52 percent of the cancers detected were stage I.

NanoSentry's core technology is cfDNA methylation pattern recognition. Tumor cells release DNA fragments with specific methylation marks into the bloodstream during apoptosis; those marks differ significantly from normal cell DNA. NanoSentry sequences roughly two million methylation sites in the blood and uses a machine-learning model to identify the methylation signatures of specific cancers.

On performance, NanoSentry achieved an overall sensitivity of 78 percent across the 14 target cancers and a specificity of 99.5 percent. For three cancers that are difficult to detect early by conventional means — pancreatic, ovarian, and esophageal — sensitivity was 73 percent, 68 percent, and 71 percent respectively. Dr. Anne-Renee Hartman, who led the research team, called the breakthrough critical to improving outcomes for these cancers.

On the business model, GRAIL announced pricing for NanoSentry in the UK at £995 per test. The NHS has decided to include it in the universal health insurance preventive screening catalogue: citizens over 50 can receive a free test every two years. The policy is expected to add roughly £450 million a year to NHS spending, but GRAIL estimates that earlier detection can cut late-stage treatment costs by around £670 million annually.

Regulatory progress in the United States is also advancing. GRAIL has filed a pre-market approval application with the FDA; the Oncologic Drugs Advisory Committee plans to convene a dedicated meeting in the second half of 2026 to discuss the product. If approved, NanoSentry will become the first US cfDNA product for multi-cancer early screening.

Some oncologists have raised concerns about overdiagnosis. Dr. Marleen Kuijper of the Netherlands Cancer Institute noted that some early cancers flagged by NanoSentry may never progress to clinical disease, leading patients to undergo unnecessary treatment. She recommended that NanoSentry reports distinguish between "definitively carcinogenic signals" and "indolent signals" to avoid imposing psychological burdens on patients.

Privacy and data use are another point of contention. NanoSentry test results are uploaded to GRAIL's cloud database and linked with patients' genomic data. GRAIL has committed to strict data privacy protection, but the UK's Information Commissioner's Office (ICO) is still reviewing whether the product complies with certain specific provisions of the GDPR.
